Only Class Members of the Purchase Class are eligible to submit a claim.
Note: If you fall into both the Purchase Class AND the Repair Class you may only participate in the Settlement as a Class Member of the Repair Class. You are not eligible to submit a claim form.
To receive a cash payment from the Purchase Class Settlement Fund Allocation, please complete this Claim Form by clicking the ‘Start Your Claim’ button below. You may also download a Claim Form here and mail it to the address on the form. The deadline to file a claim online is January 11, 2027. If you mail in a Claim Form, it must be postmarked on or before January 11, 2027.
